I was unable to find a date; at some point the hospital was re-named the Nevada Regional Medical Center. There is a spot stain at the right edge of the front cover; wear at the edges of the covers; the back cover and last few pages have a couple of bends/creases at the bottom outer corner. There is a paper clip at the top of a page. No other stains, marks, or rips inside, that I noticed when I flipped through the pages. Overall, the pages look very good. Please email with questions.
